Central Maine Orthopaedics provides the highest level of orthopaedic care, with physicians managing the entire spectrum of outpatient services from the initial appointment to the last and every question or concern in between. Appointments with our physicians can be scheduled over the phone by calling the patient accounts staff at (207) 783-1328 between the hours of 8 a.m. and 4:30 p.m.
Please plan to arrive 15 minutes prior to your first appointment with us, and bring the following information with you:
- The completed healthcare questionnaire mailed to you before your appointment date, regarding patient information, health history and current medications. If your appointment is more than one week away, please mail the completed form to CMO. Download patient forms [pdf]
- Medical insurance cards and/or Workers' Compensation information (employer's name, insurance adjuster's name, claim number and telephone number).
- Appropriate clothing for your visit (i.e. shorts for a knee problem, a sleeveless shirt for a shoulder/arm problem).
- Any related medical records should be forwarded from your referring physician. Please call and arrange prior to your appointment.
Patients should enter the CMO at the main entrance in the middle of the building.
Payment for co-pays or services not covered by an insurance company will be due at the time of the service unless alternative arrangements are made with outpatient accounts staff. We participate in most major insurance plans including Medicare and Medicaid. If you have insurance that requires a referral authorization from your primary care physician, you are responsible for obtaining that authorization prior to your appointment with us.
